That is because China Government has implemented cross-province medical data exchange system which allow employees to get medical care in anywhere China, even your local Chinese staff are recuited in Xiamen Evershine.
If you have not any entity in China, you pay salary with expenses reimbursement from overseas parent company to your local China individual employees, who will be bear risk without paying individual income tax to tax bureau. Unless your local China individual employees have setup their own company, then you pay to the company, that will be no problem.
